Quality Care<\/h3>\n

We provide patient care in accordance with regulatory requirements in a safe, clean setting. Only high quality, medically necessary care is provided.<\/p>\n

Medicare Conditions of Coverage<\/h3>\n

We are responsible for ensuring our dialysis facilities meet Medicare Conditions of Coverage for ESRD facilities. This is accomplished by developing and implementing policies and procedures, by monitoring and auditing facility performance, and by continual diligent management of each facility.<\/p>\n

Patient Rights and Responsibilities<\/h3>\n

We ensure our patients are informed of their rights and responsibilities as required by Medicare and certain other laws and rules.<\/p>\n

Confidentiality of Patient Information<\/h3>\n

We are obligated under the federal law known as HIPAA and related state privacy laws to safeguard the security of electronic patient information and safeguard the confidentiality of all patient information (\u201cPHI\u201d) in any form. We strive to safeguard patient information and prevent breaches.<\/p>\n

Relationships with Patients<\/h3>\n

While close relationships often form between patients and their healthcare providers, we avoid outside personal or business arrangements with patients. We avoid creating a situation in which you, our patients, think your care may depend in any way on whether you have a personal relationship with a provider or staff member, or whether you purchase services or products from a provider or staff member.<\/p>\n

Prohibition on Improper Inducements<\/h3>\n

IRC attracts patients based on the high quality of care we provide, our convenient locations and welcoming environments. IRC does not attract patients by routinely waiving copays and deductibles, or by providing or promising benefits, payments, gifts or other things of value. For patients who have a financial hardship, we have a program to provide financial assistance as permitted under applicable law.<\/p>\n<\/div><\/div><\/div>

CIVIL RIGHTS NOTICE TO PATIENTS<\/h3>\n

IRC complies with applicable Federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, or sex. ARA does not exclude people or treat them differently because of race, color, national origin, age, disability, or sex.<\/p>\n

ASSISTANCE FOR DISABILITIES<\/h3>\n

IRC provides free aids and services to people with disabilities to communicate effectively with us, such as (a) qualified sign language interpreters and (b) written information in other formats (large print, Braille, audio, accessible electronic formats, other formats).<\/p>\n

ASSISTANCE FOR LIMITED ENGLISH PROFICIENCY (LEP)<\/h3>\n

IRC also provides free language services to people whose primary language is not English or those who otherwise have Limited English Proficiency (LEP), such as qualified interpreters or information written in other languages.<\/p>\n

COMPLAINT PROCEDURE FOR PATIENTS<\/h3>\n

If you believe that we have failed to provide these services or discriminated in another way on the basis of race, color, national origin, age, disability, or sex, you may file a grievance or complaint.<\/p>\n
