Transitioning patients into dialysis treatment and their adherence to long-term care for end-stage renal disease (ESRD) is not without challenges. Without timely and consistent care, patients face a higher risk of fluid overload, heart problems, high potassium levels, hospitalization, and death. Modality, lifestyle and nutritional education and support are important components to keeping patients healthy as long as possible.

2. AI-supported treatment decisions 

Our proprietary artificial intelligence platform, InnovAItive, uses deep learning algorithms to identify potential health events, further personalize visits, and drive exceptional clinical outcomes. With real-time insights, the care team can intervene quickly to mitigate the risk of adverse events. As of March 2025, InnovAItive data resulted in a 20% reduction in hospitalizations and an 8% overall improvement in all key health metrics.  

3. Streamlined admissions & improved access 

Timely access to dialysis care is critical. Our teams work tirelessly to deliver streamlined admissions, day-of-discharge dialysis, assistance for underinsured or uninsured patients, and acceptance of complex cases. We also have a team dedicated to helping patients navigate insurance and Medicare coverage. Additionally, we offer all modalities of dialysis to patients, such as peritoneal and home hemodialysis to accommodate those who prefer or need to receive care outside of a center.
